In this position, you will facilitate seamless transitions for patients from hospital to home, ensuring they receive the care they need. Be part of a dedicated team committed to making a difference in the lives of our patients in Sherman Oaks, CA. This is a<strong> full-time, temporary </strong>position.</div><div>

</div><div><strong>Responsibilities:</strong></div><ul>
<li>Coordinate post-discharge care plans and follow-up appointments for patients.</li>
<li>Conduct comprehensive assessments to identify patient needs and barriers to care.</li>
<li>Collaborate with healthcare providers to ensure continuity of care and medication management.</li>
<li>Educate patients and families on discharge instructions and available resources.</li>
<li>Monitor patient progress and adjust care plans as necessary to promote recovery.</li>
<li>Document all interactions and care plans in the electronic health record (EHR) system.</li>
<li>Participate in quality improvement initiatives to enhance patient care processes.</li>
<li>Act as a liaison between patients, families, and healthcare teams to advocate for patient needs.</li>
</ul><div><strong>Requirements:</strong></div><ul>
<li>Bachelor's degree in Nursing, Social Work, or a related field preferred.</li>
<li>Current RN or LCSW license in California is required.</li>
<li>Minimum of 2 years of experience in care coordination or case management.</li>
<li>Strong understanding of healthcare systems and community resources.</li>
<li>Excellent communication and interpersonal skills for patient interaction.</li>
<li>Proficient in electronic health record (EHR) systems and documentation.</li>
<li>Ability to work independently and manage multiple priorities effectively.</li>
<li>Compassionate demeanor with a commitment to patient-centered care.</li>
