</ul><div><strong>Job Description</strong></div><div>We are seeking compassionate, reliable, and detail-oriented caregivers to assist a client transitioning home from a skilled nursing facility. This role is vital in ensuring the client remains safe, comfortable, and supported during their recovery.<br><br>
</div><div>The ideal candidate will be comfortable following a structured care plan and maintaining accurate clinical logs.<br><br>
</div><div><strong>Key Responsibilities</strong></div><ul>
<li>
<strong>Personal Care:</strong> Providing essential assistance with showering, toileting, and dressing.</li>
<li>
<strong>Mobility Support:</strong> Assisting the client in and out of bed and ensuring safety during transfers.</li>
<li>
<strong>Clinical Tracking:</strong> Recording daily vitals in a Weekly Clinical Log.</li>
<li>
<strong>Detailed Care:</strong> Following a specific Morning and Evening Daily Care Checklist to ensure all needs (nutritional, hygiene, and comfort) are met.</li>
</ul><div><strong>Requirements</strong></div><ul>
<li>Proven experience in home care or a similar healthcare setting.</li>
<li>Strong communication skills and an empathetic approach to senior care.</li>
<li>Ability to maintain clear and accurate daily documentation.</li>
<li>Reliable transportation to ensure punctuality for scheduled shifts.</li>
<li><strong>Georgia CNA.PCA, or HHA certification, preferred.</strong></li>
