Position Summary:

The LVN-Case Manager assists in the collaboration of research, development, implementation, revision and reporting of the case management program.  Working with all appropriate members of the health care team, ensures coordination of patient care through the development and use of standards of patient care and nursing practice.  Ensures that specific patient outcomes are achieved and that variances from established and accepted parameters are evaluated and addressed as necessary.


Qualifications:

Education:

      Requires completion of an accredited school for Licensed Vocational Nurse program.



Experience:

  • Prefer two years of clinical experience.
  • Working knowledge of health care reimbursement issues.
  • Effective oral and written communications skills.
  • Self-directed, self-starter able to implement new programs.
  • Able to provide education to staff, physician, and auxiliary staff, patient and family members.
  • Utilization Management experience preferred; understanding of systems and change process.
  • Computer skills and knowledge: MicroSoft: Word, Windows, Excel.
  • Electronic Medical Record Skills.



License/Registration/Certification:

  • Current California Board of Nursing LVN License.
  • Current American Heart Association BCLS certificate

Madera Community Hospital is a not-for-profit community health resource and is not associated with any other hospital or health system. The hospital is locally governed by a board of trustees which provides governance and oversight. The board is composed of over a dozen community and business leaders. Madera Community Hospital was founded in 1971 and is located at 1250 E. Almond Avenue in Madera, California. The hospital contained 106 acute care beds, a 16-bed Emergency Department, and a 10-bed Intensive Care Unit. Madera Community Hospital also operates two rural health care clinics (Mendota and Chowchilla) and a home health agency (Family Health Services in Madera).
