We’re looking for a <strong>Licensed Practical Nurse (LPN)</strong> who is ready to provide this level of care in a part-time capacity.<br><br>

<strong>Role Overview:</strong><br> The <strong>LPN</strong> will provide in-home nursing care to patients under the supervision of a Registered Nurse. You will be part of an interdisciplinary team dedicated to ensuring comfort, quality of life, and dignity for every patient and their family.<br><br>

<strong>Your Key Responsibilities Will Include:<br></strong><br>

<li>Administering medications and treatments following hospice protocols and nursing standards<br><br>
</li>
<li>Performing essential clinical tasks within the scope of LPN practice<br><br>
</li>
<li>Monitoring changes in patient condition and notifying the RN as needed<br><br>
</li>
<li>Contributing to the development and revision of care plans with measurable outcomes<br><br>
</li>
<li>Maintaining complete and timely documentation of all care activities<br><br>
</li>
<li>Offering emotional support to patients and loved ones<br><br>
</li>
<li>Participating in care coordination meetings and collaborating with the broader care team<br><br>
</li>
<li>Encouraging teamwork by supporting other staff as needed<br><br>
</li>

<strong>Ideal Candidate Qualities:<br></strong><br>
</div><ul>
<li>Graduate of an accredited LPN or LVN program<br><br>
</li>
<li>Holds an active LPN license in the State of New Mexico<br><br>
</li>
<li>CPR certification required<br><br>
</li>
<li>Possesses a valid driver’s license and auto insurance<br><br>
</li>
<li>Minimum of 1 year of nursing experience (hospice experience is a plus)<br><br>
</li>
<li>Strong interpersonal and organizational skills<br><br>
</li>
<li>A compassionate, professional approach to patient care<br><br>
</li>
<li>Comfortable with basic math and medication administration procedures<br><br>
</li>
