<strong>Position Summary<br></strong><br>
</div><div>As a <strong>Part-Time LPN</strong> at Luna Del Valle Hospice, you will deliver direct patient care in the home setting, under the supervision of a Registered Nurse (RN). This role is essential to ensuring high-quality, compassionate care that respects patient dignity and supports families during a vulnerable time.<br><br>
</div><div>
<strong>Responsibilities<br></strong><br>
</div><ul>
<li>Administer medications and treatments per Hospice policies and clinical standards<br><br>
</li>
<li>Perform baseline nursing skills with clinical competency and in accordance with state practice acts<br><br>
</li>
<li>Monitor patient condition and promptly report any changes, needs, or concerns to the supervising RN<br><br>
</li>
<li>Assist in developing and updating patient-centered Plans of Care, including measurable goals and interventions<br><br>
</li>
<li>Maintain accurate and timely documentation of nursing interventions, observations, and education provided<br><br>
</li>
<li>Provide emotional support to patients and families, always upholding patient rights and dignity<br><br>
</li>
<li>Communicate effectively with physicians, interdisciplinary team members, patients, and families<br><br>
</li>
<li>Participate in team conferences and contribute to collaborative care planning<br><br>
</li>
<li>Promote teamwork by supporting and assisting fellow staff members when needed<br><br>
</li>

<strong>What You Bring<br></strong><br>
</div><ul>
<li>
<strong>Graduate</strong> of an accredited Licensed Vocational/Practical Nursing Program<br><br>
</li>
<li>
<strong>Current and valid LPN license</strong> in the State of New Mexico<br><br>
</li>
<li>
<strong>CPR certification</strong> and a valid <strong>driver’s license with auto insurance<br></strong><br>
</li>
<li>At least <strong>1 year of nursing experience</strong> (Hospice experience preferred)<br><br>
</li>
<li>Strong organizational, time management, and communication skills<br><br>
</li>
<li>A cooperative, compassionate approach to patient care<br><br>
</li>
<li>Ability to handle emotional demands with grace and professionalism<br><br>
</li>
<li>Basic math skills for medication and treatment administration<br><br>
</li>

<li>Maintains patient confidentiality and adheres to HIPAA and privacy policies<br><br>
</li>
<li>Participates in continuing education and in-service trainings<br><br>
</li>
<li>Keeps current with health care regulations and standards<br><br>

<li>May be required to participate in occasional on-call rotation<br><br>
