<div><strong>About the Role:</strong></div><div>Join Love Joy Adult Day Health Care Center as a Social Worker, where you will play a crucial role in enhancing the well-being of our clients. We are dedicated to providing compassionate care in a supportive environment, making a difference in the lives of those we serve.</div><div>

</div><div><strong>Responsibilities:</strong></div><ul>
<li>Conduct assessments to determine clients' social, emotional, and psychological needs.</li>
<li>Develop and implement personalized care plans in collaboration with clients and their families.</li>
<li>Provide counseling and support to clients and their families to navigate challenges.</li>
<li>Coordinate with healthcare providers to ensure holistic care delivery.</li>
<li>Facilitate support groups and educational workshops for clients and caregivers.</li>
<li>Maintain accurate and confidential client records in compliance with regulations.</li>
<li>Advocate for clients' rights and access to community resources.</li>
<li>Stay updated on best practices and trends in social work and elder care.</li>
</ul><div><strong>Requirements:</strong></div><ul>
<li>Master's degree in Social Work (MSW) or other related field<ul><li> or Bachelors degree in Social Work or other related field and 2 years of experience in a healthcare or social services setting</li></ul>
</li>
<li>Strong communication and interpersonal skills with a compassionate demeanor.</li>
<li>Ability to work collaboratively in a multidisciplinary team environment.</li>
<li>Knowledge of community resources and services for seniors.</li>
<li>Proficient in documentation and familiarity with electronic health records.</li>
<li>Commitment to ongoing professional development and ethical practice.</li>
