Lodge an industrial relations matter
Our online forms are designed to enable you to lodge industrial relations matters easily through the Digital Registry.

Commission forms are made available for online lodgement or a downloadable PDF file.
To submit your form online, you will need to create a profile in the Digital Registry. This will allow you to lodge and track your matters online.
If you prefer to file your forms by email or post, you may download the PDF version of the form, by selecting download within the 'More Information' section of the required form.
IAC forms are currently only available for download from the Digital Registry and IMC forms are currently only available for download from the IMC website.
