TITLE: Home Hospital Teacher
QUALIFICATIONS: As set by state certification authorities
REPORTS TO: Superintendent or designee
SUPERVISES: Student activities of the program
JOB GOAL: To provide an appropriate educational program for students who are receiving home hospital educational services.
PERFORMANCE RESPONSIBLITIES: 1. Instruct students enrolled in the homebound program according to state, federal, and local guidelines.
2. Make provisions to be available to students and parents for educationally related purposes outside the instructional day when requested to do so under reasonable terms.
3. Coordinate student instruction and grade/progress reporting, student record maintenance as needed.
4. Evaluate and grade student progress on a regular basis.
5. Obtains student and curriculum information from all necessary school personnel.
6. Works directly with the teacher(s) and administrators to plan a program of instruction for each student enrolled.
7. Maintain accurate, complete, and correct records and reports as required by board policy, state, and federal regulations.
8. Attends and participates in educationally-related meetings.
9. Assists in testing and evaluating each student within the program.
10.Makes referrals and guides parents or student to appropriate agencies for needed services.
11.Works to establish and maintain open lines of communication with students and their parents concerning both the broad academic and behavioral progress of all assigned students.
12.Provide for professional growth through an ongoing program through workshops, seminars, and conferences; and/or advanced coursework at institutions of higher learning.
13.Attends and participates in faculty meetings.
14.Accepts a share of responsibility for co-curricular activities.
15.Establishes and maintains cooperative relations with others.
16.Performs other duties as assigned.
