1. Introduction
Larsen & Toubro Public Charitable Trust (LTPCT) is working at Vikramgad of Palghar districts of Maharashtra state for overall development of this aspirational block since last 10 years. LTPCT's various programs and initiatives in Vikramgad are well aligned to SDGs (Sustainable Development Goals) adopted by United Nations. Over the period, LTPCT's various initiatives in the thematic area of Health, Education, Agriculture, Livelihood, Skill Development has been to strengthen local resources and work towards sustained reforms for existing ecosystem and improving accessibility of health and livelihood development services to local community. Since LTPCT is working in Talwada & Malwada blocks (Vikramgad) for quite a long time, it is well versed with the culture and socio-economic-educational traits of the poplulation living in the area.
Larsen & Toubro Public Charitable Trust plans for implementing "Sickle Cell Disease Control Program" with objective of detection & control of the sickle cell disease. Project aims to detect cases with sickle cell disease in 3 PHCs of Vikramgad block of Palghar District, Maharashtra.
This Request for Proposal (RFP) invites prospective organizations to submit proposals for implementing a Sickle Cell Control Program in the Vikramgad block. The RFP provides organizations with relevant operational, performance, application, and architectural requirements that the system must fulfill. This document outlines the Request for Proposal (RFP) issued by Larsen & Toubro Public Charitable Trust (LTPCT). Please note that this document is confidential and proprietary. Organizations invited to submit proposals are expected to maintain the confidentiality of the information contained within this document.
1.1 Purpose of RFP
Sickle cell disease is a genetic blood disease which affects the whole life of affected patient. It is more common in the tribal population of India but occurs in non-tribals too. It not only causes anaemia but also pain crises, reduced growth, and affects many organs like lungs, heart, kidney, eyes, bones and the brain. India has the largest density of tribal population, globally. As per Census 2011, India has an 8.6% tribal population which is 67.8 million across the Indian states. The MoHFW tribal health expert committee report has listed sickle cell disease as one of the 10 special problems in tribal heath that affect the tribal people disproportionately, thus making this an important intervention. Ministry of health under NHM initiated the work on hemoglobinopathies (Thalassemia & Sickle Cell Disease) in 2016 wherein comprehensive guidelines on prevention and management of hemoglobinopathies were released and provision of funds towards screening and management of Sickle cell disease were made. Palghar District, a predominantly tribal area, is grappling with the iceberg phenomenon of sickle cell disease, where many cases remain undetected due to limited and opportunistic screening. This program aims to conduct comprehensive screening for individuals aged 1 to 40 across the entire Vikramgad block. In addition to early detection, the program will facilitate access to treatment, provide counselling, and offer rehabilitation services for beneficiaries. Furthermore, the program will work towards raising awareness about sickle cell disease within the community, ensuring better understanding and support for affected individuals.
1.2 Scope of Work
To execute mass screening with diagnosis at point of care for sickle cell disease supporting government efforts to detect the cases of sickle cell disease.
To control genetic transmission of Sickle cell provision of one-to-one counselling
To implement elements of National Sickle cell elimination program at ground level – like facilitation for treatment, Distribution of cards, Rehabilitation of diseased etc.
1.3 Objectives
i. To detect the cases of Sickle cell disease & trait through screening of 1-40 age population. (25000 Population per year)
ii. To prevent transfer of Sickle cell disease gene by Premarital & Prenatal counselling to the detected traits. (Target – 2 Counselling session in a years/ detected traits)
iii. To facilitate linkage to government schemes (Sanjay Gandhi Niradhar Yojana, Disability Certificate) to diseased. (Target SGNY – 95% Coverage Disability
Certificate – 95%) iv. To create awareness amongst community regarding sickle cell anaemia through IEC, BCC focusing to reduce the stigma on Sickle cell disease (Target – 24 Awareness session at community level – 8/PHC)
1.4 Deliverables
Mass Screening of 25000 population age 0-40 under 1 PHC in Vikramgad block.
Ensuring provisionally positive in mass screening gets tested at PHC by confirmatory test & maintaining data.
24 Community awareness sessions.
Approx. 2000 Counselling sessions for diagnosed traits & diseases.
Issuing cards to the traits & diseased.
Ensuring treatment provision by government facilities
Ensuring enrollment of 120 Positive diseased under Sanjay Gandhi Niradhar Yojana
Ensuring 120 positive diseased gets disability certificate from government.
1.5 Timeline of the project
The project is scheduled to start in June 2025 and will conclude in March 2026, with potential continuation based on satisfactory performance.
