- Hours: 8:30 a.m. -5:00 p.m. Monday- Friday
- Weekend on call every six (6) weeks

Position Description:
The Community Based Palliative Care Nurse Practitioner functions as an integral member of the CBPC team, and works in collaboration with Kansas City Hospice and Palliative Care (KCH&PC) staff to provide comprehensive, quality hospice and palliative care services to patients. The Nurse Practitioner acts as the Palliative Care Case Manager, and is responsible for providing patient visits and coordinating their plan of care, for both new and established palliative care patients, in patient homes and facilities. The Nurse Practitioner engages in independent decision making regarding a patient’s healthcare concerns and works with autonomy, within their scope of service, under the medical oversight of patients’ Community Based Physicians and KCH&PC’s Medical Director.
The Ideal Candidate Will Possess the Following Requirements:
- Appropriate Masters Degree leading to designation as Advanced Practice Nurse
- Current Kansas and Missouri nursing license.
- One year acute care experience preferred.
- Home Health or Hospice experience preferred.
- Licensed driver with reliable transportation and auto insurance.
- CPR certification required.
