About the role

Direct Services to Clients • Identify new clients; negotiate with them to enroll them in the intervention programs of project Naya Daur. • Visit all clients of the assigned field each day to provide food, medicine, hygiene items, and clothes. • Ensure improvement in the overall health status and self-care of existing clients under your assigned fields. • Provide support in developing treatment planning for clients assigned under your field. • Conduct group activities jointly with Counsellor. • Facilitate Doctor's visit to clients and interact with the doctor regarding their status. • Ensure Counsellor's visits at least twice a month in your assigned fields. • Ensure quarterly scale assessment of all clients under your care. • Organize hospitalization or institutional care of client/s if needed. Visit for follow-up on the regular basis for the admitted client/s. • In case of any concern, bring it to the attention of the Project Coordinator, Program Officer, and Assistant Directors and concerned Doctor. • Attend to emergency situations when necessary. Building Care Network & Awareness • Increase the number and involvement of caregivers in your assigned field. • Motivate people from community to attend and provide care and support for at least one homeless person with psychosocial disability. • Conduct and participate in caregiver's capacity building. • Motivate NGOs and other related organizations to provide shelter support to the homeless person with psychosocial disability. • Participate actively in organizing awareness camps, medical camps in your project area according to the Annual Work Plan of the project. • Provide assistance in organizing rehabilitation fair, sports, and other awareness generating events as per need. • Identify potential caregivers, encourage them to engage in regular caregiving services, regularly interact with them to ensure retention. Advocacy • Provide support in identifying and establishing professional relationship with police officials, Govt. health service providers, local NGOs & CBOs, KMC ward councilors, concern Govt. departments to assist clients gain access to education, care, entitlements and support services Reintegration and Inclusion of Clients • Motivate clients to be economically active, conduct skills development training and organize supportive employment for them in the community if they are capable and willing. • Regularly interact with employers for retention and equal rights. • Facilitate Govt. entitlements, community-based resettlement. • Provide assistance to the Restoration Team for client restoration/ resettlement, pre-post restoration visits as needed. • Visit the homes and families of restored patients for follow-up and maintain communication with the assigned families. Documentation and Reporting • Prepare and maintain clients' report, clients' register, scale administration report, MIS, • Submit medicine requisition in timely manner; maintain medicine disbursement report, day sheet, and monthly plan. • Filling up of LSP sheets every month. • Maintain daily diary of field work. Teamwork, Inter Team Support & Support to the organization as a whole • Provide support to other project teams as required. • Attend various meetings as scheduled e.g. staff meetings, monthly meetings, shelter meetings etc. • Participate in event committee meetings and assist in organizational events. • Participate in various trainings and workshops. • Participate in various outdoor activities of projects. • To carry out other duties as may reasonably be required from time to time.
