Primary Functions:
Under the direction of the Director of Nursing, evaluates patient’s eligibility for HPC programs, instructs patients and families in infusion therapy administration, and delivers direct patient care as required.
Major Responsibilities:
- Conducts initial nursing assessments and evaluations to qualify patients for appropriateness and safety of home infusion therapy, along with any other needed specialized nursing care and educational needs
- Trains patients and/or caregivers in all procedures and equipment
- Develops and revises the patient’s plan of care with the patient, family and physician
- Visits all assigned patients as ordered, and documents client’s compliance
- Provides direct patient care as ordered and allowed by state law
- Inventories patient factor and supplies, informs pharmacy of patient’s needs
- Coordinates patient’s care with other homecare agencies as necessary
- Participates in the identification of nursing CQI indicators and the collection of data
- Medical treatment plans, physician certifications, and other required reimbursement documents
- Provides other healthcare workers with feedback on all patients and gives suggestions for changes in procedure/protocols
