<strong>Position Summary<br></strong><br>
</div><div>The <strong>Registered Nurse (RN) Care Manager</strong> is responsible for coordinating, implementing, and overseeing personalized care plans for clients receiving in-home support services.<br><br>
</div><div>This role ensures that clients receive <strong>high-quality, person-centered care</strong> while maintaining their overall health, safety, and well-being. The Care Manager serves as the <strong>primary point of contact</strong> between clients, families, caregivers, and healthcare professionals, ensuring services are delivered effectively and in compliance with all regulatory and company standards.<br><br>
</div><div>This position requires <strong>strong clinical judgment, organization, communication, and leadership skills</strong>, along with the ability to manage multiple priorities in a fast-paced environment.<br><br><strong>Minimum Qualifications<br></strong><br>
</div><ul>
<li>Active <strong>Registered Nurse (RN) license</strong> in the State of California (Required)</li>
<li>Minimum <strong>2 years of experience</strong> in home care, home health, hospice, or related field</li>
<li>Valid driver’s license, reliable transportation, and current auto insurance</li>
<li>Excellent written and verbal communication skills</li>
<li>Strong attention to detail, follow-up, and organizational skills</li>
<li>Ability to problem-solve, take initiative, and exercise sound clinical judgment</li>
<li>Experience supervising caregivers and advocating for clients and families</li>
<li>Professional, positive, and client-centered demeanor</li>
<li>Ability to pass a criminal background check</li>
<li>Bilingual (English/Spanish) is a plus</li>

<strong>Caregiver Oversight & Support<br></strong><br>
</div><ul>
<li>Supervise caregivers to ensure:<ul>
<li>Compliance with care plans</li>
<li>Proper infection control practices</li>
<li>Timely reporting of changes in client condition</li>
<li>Adherence to emergency protocols</li>
</ul>
</li>
<li>Provide coaching, guidance, and support to caregivers</li>
<li>Ensure caregivers have the appropriate skills for each case</li>
</ul><div>
<strong>Client & Family Coordination<br></strong><br>
</div><ul>
<li>Serve as the primary point of contact for clients and families</li>
<li>Facilitate caregiver introductions for first shifts</li>
<li>Review care plans with clients and caregivers and set expectations</li>
<li>Ensure care plan documentation is accessible in the client’s home</li>

<strong>Operational Responsibilities<br></strong><br>

<li>Assist with caregiver scheduling coordination</li>
<li>Collect fees for initial services at the time of assessment or prior to the first day of service</li>
<li>Ensure compliance with company policies and regulatory requirements</li>

<strong>Compensation & Bonus Structure (Premium Tier)<br></strong><br>
</div><ul>
<li>
<strong>Pay Range:</strong> $50 – $70 per hour (based on experience, clinical background, and leadership capability)</li>
<li>
<strong>Monthly Performance Bonus:</strong> $750 – $3,000+<ul><li>Based on client outcomes, care plan compliance, documentation quality, and family satisfaction</li></ul>
</li>
<li>
<strong>Case Load Bonus:</strong> Tiered incentives for managing higher-acuity or higher-volume caseloads</li>
<li>
<strong>Referral Bonus:</strong> $300 – $750 for each new client referral that starts services</li>
<li>
<strong>Retention Bonus:</strong> $150 per client retained beyond 90 days</li>
<li>
<strong>Leadership Incentive (Senior/Lead RNs):</strong> Additional $1,000 – $3,000/month based on team oversight and performance metrics</li>
</ul><div>Total compensation is performance-driven and scales with caseload, leadership contribution, and client outcomes.</div><div><br></div><div>
