Role Description
We are seeking a dedicated and compassionate Home Health Care Coordinator to join our team. In this role, you will be responsible for coordinating and overseeing home care services across the organization, ensuring seamless service delivery and client satisfaction.
The Home Health Care Coordinator will work closely with healthcare providers, patients, families, and other members of the care team to ensure the delivery of high-quality, patient-centered care.
The ideal candidate will possess strong coordination abilities, a solid clinical understanding, excellent communication skills, and a genuine passion for improving patient outcomes and overall well-being.
Responsibilities
-Coordinate and manage end-to-end home care services for patients.
-Act as the primary point of contact for patients, families, and healthcare providers.
-Assess patient requirements and match them with suitable caregivers.
-Maintain regular communication with patients and families to track satisfaction and address concerns.
-Coordinate with vendors, nurses, and attendants for staffing and replacements when required.
-Ensure compliance with company protocols, healthcare standards, and service agreements.
-Handle escalations, resolve issues promptly, and ensure continuity of care.
-Support billing, invoicing, and payment follow-ups in coordination with the finance team.
-Ensure proper onboarding and briefing of caregivers before deployment.
Qualifications
-Bachelor’s degree in Healthcare Administration, Nursing, Life Sciences, or a related field.
-Prior experience in home healthcare, hospital coordination, or patient care management is preferred.
-Strong understanding of home care services, patient needs, and caregiver management.
-Excellent communication and interpersonal skills.
-Ability to handle multiple cases and manage operations efficiently.
-Strong problem-solving skills with the ability to handle escalations.
-Proficiency in MS Office (Excel, Word) and basic documentation tools.
-Ability to work in a fast-paced and dynamic environment.
-Compassionate, patient-focused approach with high levels of empathy.
-Strong organizational and time management skills.
-Willingness to be available for coordination and support as per business needs.
