High Desert Medical Group is seeking a full time LVN to work as a Case Management Clinic Nurse. The employee will be responsible for providing direct patient care to Complex Case Management/High Risk Clinic patients.
Summary of Duties:
- Manage/monitor patients, including but not limited to: daily/weekly phone calls, home visits and/or PCP office visits.
- Create and maintain working files/logs and/or tracking systems for each patient to ensure proper follow-through.
- Follow up on patients by making necessary calls to patients/families/providers to optimize continuity of care and minimize resource utilization and hospitalizations.
- Utilize in-house, social service, health plan, and community resources available to provide appropriate education and care delivery required to optimize patient health and welfare.
- Communicate with provider and staff to ensure necessary required paperwork is available for review by provider before or at time of patient appointments.
- Communicate with physician/provider and nursing staff to ensure appointments are being kept.
- Coordinate with appointment scheduler to ensure patients are being scheduled appropriately, and in a timely manner.
- Assist in generating needed referral for patients in order to expedite care, optimize health, and/or prevent exacerbation of disease process.
- Function as liaison between in-patient and out-patient delivery systems to facilitate continuity of care.
- Assist in documentation of non-compliance for processing of health plan notification.
- Assist patients to exam rooms and obtain complete and accurate vital signs.
- Obtain and document all pertinent information in patient's charts.
- Anticipate, assist, and perform procedures ordered by the provider.
- Maintain timely patient flow and room availability.
- Maintain patient safety and confidentiality.
- Reinforce patient education and participate in patient teaching.
- Stock and maintain exam rooms, nursing stations, and individual workstations.
- Participate in unit duties such as cleaning and sterilizing instruments and ordering supplies.
- Maintain a current, accurate surgical log, procedure log, lab log, and patient tickler files as appropriate.
- Maintain and monitor all equipment used for patient care and testing. Report any discrepancies to supervisor.
- Assist with training and orientation of new personnel.
- Communicate with PCP after patients are discharged from the clinic to ensure continuity of care.
- Actively participate in nursing QI by identifying problems, providing solutions, and evaluating outcomes.
- Ensure appropriate handling and accurate documentation of medications ordered by the provider.
- Able to initiate and appropriately participate in Code White, Code Blue, and Code Red within the guideline of scope of practice.
- Knowledgeable with referral system and ensures accurate and timely follow through with all referrals generated.
- Knowledgeable of HDMG computers and able to use them for job duties.
- Universal precautions utilized in all patient care.
- Assume responsibilities for narcotic medications as assigned.
- Maintain crash cart, emergency medications, and equipment as needed.
- Perform other duties as assigned.
The pay range for this position at commencement of employment is expected to be between $29.00 – $38.67/hour, non-exempt; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
