Position Overview:

Essential Duties and Responsibilities:

  • Provide habilitative individual and family/psychotherapy and psycho educational skill building; micro and macro levels of advocacy;
  • Facilitate resource mapping;
  • Utilize DSM-V and ICD-10 codes for diagnostic and billing purposes
  • Participation in multi-disciplinary teams of care;
  • Connect with individuals who have so much to teach you about your practice.
  • Commit to professional growth through group and/or individual supervision
  • Attend trainings to stay abreast of best practice in clinic and population support
  • Embrace Mandated Reporter and Duty to Warn Status
  • Participate in evaluation and visioning processes to grow the department and agency.
  • Contemporaneously document services, complete assessments and reports in a timely manner.
  • Willingness to familiarize self, operate within, and adhere to NYS DOH, NYS ED, and NYS OPWDD regulations
  • Professionally represent client and/or HCA while interfacing with community agencies, coalitions, etc.
  • Maintain NYS Licensure
  • Maintain professional boundaries and design self-care plan
  • Ability to manage daily scheduling and goal-oriented planning.
  • Confidence in administering Mental Health First Aid, Suicide Risk Assessments, Contracting for Safety.
  • Prioritize and foster agency and autonomy of client
  • Operate from a strengths-based perspective enacted through person-centered visioning.
  • Embrace technology: proficiency in Electronic Medical Records, Microsoft suite, Outlook, telehealth platforms
  • Attend mandatory trainings.
  • Ability to be flexible with daily schedule based on the needs and requests of the consumer.
  • Maintain working knowledge of OPWDD requirements.
  • Other duties or programs such as group facilitation as assigned.

Requirements

Education and Experience Requirements:

  • MSW from a nationally accredited institution 
  • NYS LMSW, LCSW, or L-CSW-R, or NYS Limited Permit and pass the exam within 6 months of hire.

Qualifications and knowledge:

  • Excellent verbal, written and active listening skills. · Excellent organizational skills. · Ability to utilize clinical skills to resolve crisis/emergent scenarios.
  • Knowledge of social work practices and social work ethics.
  • Knowledge of the NYS Office of People with Developmental Disabilities (OPWDD).
  • Knowledge of diagnostic and treatment services including occupational therapy, physical therapy, speech therapy, and special education.
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests.
  • Have access to reliable transportation and possess a valid NYS Driver’s license. · Proficient with Microsoft programs and electronic health records.

While performing the duties of this job, the employee is:

  • Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
  • Occasionally required to reach with hands and arms. 
  • Fingering is required regularly for typing and the use of hands in a repetitive motion.
  • Regularly required to talk and hear with the ability to exchange and receive detailed information
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, ability to adjust focus to perform activities such as preparing and analyzing data, viewing computer screens.
  • Working conditions of this position do not substantially expose workers to adverse environmental conditions and are typical of an indoor work environment.
