In this vital role, you will be responsible for providing expert pharmaceutical care, ensuring medication safety, and supporting patient health outcomes across diverse clinical settings. Your expertise will help optimize medication therapy, educate patients and staff, and uphold the highest standards of pharmacy practice. This position offers an exciting opportunity to make a meaningful impact on patient care while working in a collaborative and fast-paced environment.<br><br>Duties<br><br>Dispense prescription medications accurately and efficiently, ensuring compliance with all legal and safety standards.<br>Conduct thorough patient assessments to determine appropriate medication therapies, including reviewing medical histories and current treatments.<br>Collaborate with healthcare providers to develop and modify medication plans, especially in specialized areas such as LTC (Long-Term Care), critical care, pediatrics, and acute care settings.<br>Manage pharmacy inventory meticulously, including ordering, stock rotation, compounding medications, and maintaining proper storage conditions.<br>Utilize pharmacy software and electronic health record (EHR) systems to document prescriptions, patient interactions, and medication administration details.<br>Provide immunizations and perform aseptic techniques for sterile compounding or IV infusion preparations following strict protocols.<br>Educate patients on medication usage, potential side effects, and medical terminology to promote understanding and adherence to treatment plans.<br>Support medical management initiatives by participating in pain management programs, medication reconciliation, and patient care coordination.<br>Assist pharmacy technicians with workflow oversight, medication preparation, and ensuring compliance with HIPAA regulations for patient privacy.<br>Perform IV insertion procedures when necessary for medication administration or infusion therapy in hospital or clinical settings.<br>Requirements<br><br>Valid pharmacist license in good standing with relevant licensing boards.<br>Proven experience in pharmacy practice with a focus on LTC pharmacy or critical care environments preferred.<br>Completion of a pharmacy residency program or equivalent clinical pharmacy experience is highly desirable.<br>Strong knowledge of physiology, anatomy, medical terminology, and pharmacology principles essential for effective patient assessment.<br>Hands-on experience with pharmacy software systems (EMR/EHR), inventory management tools, and sterile compounding techniques.<br>Demonstrated ability to administer immunizations and perform IV infusions following aseptic techniques.<br>Familiarity with hospital or acute care settings including pain management protocols and medical management strategies.<br>Excellent communication skills for patient service delivery and interdisciplinary collaboration.<br>Ability to handle sensitive information in compliance with HIPAA regulations while maintaining confidentiality.<br>Experience working with pharmacy technicians in a team-oriented environment is advantageous.
