Requirements
- Bachelor’s Degree in Social Work by accredited college/university required in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation, counseling, or psychology.
- Must hold current limited or full license to practice in Michigan without conditions, limitations or restrictions.
- Work requires a minimum of six to twelve months on the job experience to acquire necessary familiarity with department and facility policies and procedures and to learn community resources in order to perform duties and responsibilities as noted herein.
- Approved CPR certification
Summary
- Completes psychosocial admission assessments, including evaluation of psychosocial and emotional functioning, events precipitating admission, identification of patient needs, limitations, and strengths.
- Complies with The Joint Commission (TJC) and Center for Medicaid and Medicare Services (CMS) standards with regards to the quality of documentation and time frames for completions of assessments and treatment documentation. Keeps current on State and Federal regulations, as well as professional standards and makes recommendations, as appropriate, concerning changes needed in policy and procedures.
- Coordinates the development and implementation of individualized treatment plans in consultation with the patient and other treatment team members; participates in plan reviews to determine effectiveness and revises the treatment plan accordingly in weekly team/patient care conferences. Ensures awareness of insurance coverage and estimated length of stay are appropriate for patient specific diagnosis and recovery needs.
- Gathers information regarding the patient; family dynamics, background, current situation, etc. from relevant sources. Maintains timely communication with patient’s family/significant relations and is a liaison between family, facility and community resources.
- Provides intervention services including crisis intervention, individual, and family therapies in relation to wellbeing related to illness, disability, and hospitalization. Identifies/makes recommendations for alternate treatment methods as necessary.
- Coordinates discharge plans with the interdisciplinary team and community resources, serving as a patient advocate/case manager for securing community support/further care, provides education for patient and families/significant relations, ensures discharge forms are complete (including releases or waivers) and delivered to the appropriate aftercare providers.
- Assesses the need for staff development programs in relation to psychosocial needs of the rehabilitation patients and their support systems.
- Maintains professional knowledge through participation with continuing education to provide care appropriate to the age of the patients served in the therapy setting. He/she must demonstrate knowledge of the principles of growth and development through the life span and possess the ability to assess data reflective of the patient's changing status.
- Performs additional assignments as requested by the MRC Executive and/or Director of Nursing, or as may be required by changes in current Federal, State or regulatory agency requirements/guidelines.
