What You’ll Do (AKA: Your Superpowers )

Support Clinical Excellence:

  • Accurately and timely completes position specific core roles and responsibilities that impact overall quality of care, resident, family, and satisfaction.
  • Follows all community policies and procedures related to investigating, reporting, and preventing abuse of vulnerable adults.
  • Leads and/or participates in quality improvement efforts in the community or with corporate initiatives as requested.
  • Assist DOW in reviewing daily census to ensure that residents nursing needs are met by assigning a sufficient number of staff for each shift in accordance with the budget.

Support with Building a Strong Team:

  • Counsels and coaches team members and initiates disciplinary action, as necessary.
  • Acts as a mediator between team members.
  • Encourages open communication between team members on all shifts and cooperative relationships with other departments.
  • Ensure that all team members understand the resident care plan, how to use equipment, and how to perform assigned duties, by evaluating team member competencies, documenting findings and reporting areas of concern/lack of competency to Director of Wellness.
  • Serves as a team player with an interest and enthusiasm for the total functioning of the community.

Support Regulatory Compliance:

  • Ensure residents’ charts are updated, accurate and free of errors.
  • Openly communicate and collaborate with the Director of Wellness, third party vendors and resident’s physician on changes in residents’ condition, allergies, medications to assist in resident achieving health goals.
  • Assist with development and teaching continuing education meetings and opportunities for team members and residents.
  • Participates in community and other public education regarding care and services provided at the community.
  • Supports and advocates for resident rights and promotion of resident independence, privacy, dignity, and autonomy.

Collaborate Across Departments:

  • Partner with other department leaders to support interdisciplinary communication and resident-centered care.
  • Participates in director on-call rotation per community policy and will work weekends and holidays according to nursing schedule.
  • Work closely with the Executive Director and corporate teams on staffing, budgeting, and strategic planning.
  • Participate in care conferences, family meetings, and community events as a clinical leader and advocate.

Support Resident Wellness & Satisfaction:

  • Build strong relationships with residents and families to ensure their needs and concerns are heard and addressed.
  • Foster a culture that promotes dignity, independence, and holistic care.
  • Ensures that residents receive the best care possible by encouraging open communication with the management team, direct care personnel, residents, and families.

What You Bring to the Table (Besides Your Clinical Expertise)

Education & Licensure:

  • High School diploma or GED
  • Must have and maintain an active Medication Aide Certification per state guidelines and in certain states be in good standing on the state registry.
  • Current CPR/first aid certification.

Experience & Leadership:

  • Medication Aide with minimum of 2 years of experience in long term care, geriatric care, or related field. Resident Care Coordinator Experience preferred.
  • Proven ability to manage staffing, scheduling, training, and clinical compliance.

Skills & Competencies:

  • Working knowledge and understanding of State, Federal and Local regulatory rules related to operations of Assisted Living that apply to the community.
  • Excellent communication, organization, and problem-solving skills.
  • Ability to lead by example and foster a positive, team-oriented culture.

Schedule:

  • Full-Time, Monday - Friday; 8:00am - 4:30pm*

*On-call after hours and some weekends/holidays required.
