Role Description This is a full-time, on-site HCC Coder role based in Hyderabad. The HCC Coder will review medical records and documentation to assign accurate Hierarchical Condition Category (HCC) and related diagnostic codes in accordance with established guidelines and payer requirements. The role involves validating medical necessity, ensuring appropriate capture of chronic conditions and risk adjustment factors, and maintaining compliance with coding standards and regulatory requirements. The HCC Coder will collaborate with clinical and billing teams to clarify documentation, support accurate reimbursement, and identify opportunities for coding and documentation improvement. The position also includes maintaining up-to-date knowledge of coding rules, payer policies, and industry best practices.
Qualifications
- Coding experience with a focus on HCC and risk adjustment, including accurate assignment of diagnostic codes
- Formal education or certification in Health Information Management and credentials such as RHIT
- Strong knowledge of Medical Terminology to interpret clinical documentation
- Proficiency in Medical Coding standards, guidelines, and payer-specific requirements
- Attention to detail, analytical skills, and commitment to data accuracy and compliance
- Ability to work collaboratively with clinical, billing, and compliance teams
- Familiarity with electronic health record (EHR) systems and coding software
- Relevant coding certifications (e.g., CPC, CRC, CCS) are highly desirable
