<div>Hands On Health Associates, LLC is an outpatient behavioral health program located in Brooklyn, NY looking for a full-time Russian-speaking CASAC/CASAC-T Counselor who will provide outpatient substance use treatment services to clients to enhance their recovery.<br><br>
</div><div>Qualifications</div><ul>
<li>CASAC or CASAC-T (Required)</li>
<li>EHR documentation skills</li>
</ul><div>Responsibilities and Duties</div><ul>
<li>Provide care to adults with a diverse range of issues, such as substance use, functional impairments and psychosocial needs (homelessness, justice involvement and unemployment)</li>
<li>Assist in implementing the established program policies, mission and procedures as they relate to the program.</li>
<li>Maintain a caseload and provide substance use assessments, counseling, referrals, intervention services and perform scheduled tasks as assigned.</li>
<li>Intervene with patients regarding special problems, as needed.</li>
<li>Assist in facilitating intake and orientation of new patients.</li>
<li>Maintain accurate up-to-date clinical record keeping in compliance with regulations.</li>
<li>Contributes input for Performance Improvement with regard to quality of care and planning clinical services</li>
<li>Effectively provide for crisis intervention with patients and/or family</li>
<li>Assist in development and implementation of program goals.</li>
<li>Conduct outreach to other agencies/communities and outreach absent patients.</li>
<li>Assist in coordinating services with relevant service agencies for individual patients; refer patients to other programs as needed.</li>
<li>Follow up with psychiatrists and/or attending physicians when relevant and appropriate.</li>
<li>Utilize validated tools to screen and assess patients for depression, trauma, substance use, risk and needs associated with recovery and relapse prevention.</li>
<li>Evaluate, on a continuing basis, use of services and expected outcomes for patients.</li>
<li>Provide culturally competent services in accordance with Hands On Health policies and procedures.</li>
<li>Develop group curriculum and facilitate or co-facilitate group psychotherapy.</li>
<li>Develop recovery-oriented treatment plans and reviews in collaboration with clients and consented stakeholders such as significant others, criminal justice personnel, primary health care providers and other service coordinating agencies.</li>
