At Guardian Angel Hospice, we seek a compassionate and dedicated LPN who understands the importance of hospice care. As an LPN, you will play a key role in providing specialized nursing services based on the individualized care plan created by the physician and interdisciplinary team. The right candidate will inspire confidence as they support and educate patients and their families, all while safeguarding confidentiality.
Key Responsibilities:
- Deliver services in compliance with organizational policies, procedures, and the regulations that define the scope of practice.
- Prepare clinical and progress notes detailing the specialized nursing services provided.
- Assist the RN and/or physician with specialized duties related to end-of-life care.
- Support the RN in implementing the plan of care.
- Prepare equipment and materials for treatment while adhering to aseptic techniques as required.
- Help patients learn appropriate self-care techniques.
- Assess patient and family responses to care.
- Ensure that relevant information is communicated to the appropriate team members.
- Submit documentation according to agency policies.
- Attend in-service programs and interdisciplinary team (IDT) meetings.
Job Requirements:
- Reliable transportation and a valid driver’s license.
- Current LPN state licensure.
- Strong scheduling and management capabilities.
- The ability to make sound nursing judgments in diverse field settings.
- Proficiency in assessing patient needs and creating tailored care plans.
