About the role

The National Case Manager - Case Management Services will be responsible for carrying out all aspects of case management for our multi-employer health and wellness programs across Canada. This role will work closely with both internal and external stakeholders, from plan members to third party service providers, to ensure that all case management activities are completed accurately and in a timely manner. Responsibilities: - Conduct intake calls, assess member eligibility, and determine the appropriate level of case management support - Provide proactive and responsive case management services to members, including but not limited to scheduling and coordinating appointments, arranging transportation, and facilitating referrals to appropriate community-based services - Maintain detailed case notes and ensure all documentation is complete and up-to-date - Liaise with third party service providers, such as physiotherapists and occupational therapists, to monitor member progress and ensure alignment with treatment plans - Participate in interdisciplinary team meetings to discuss complex cases and identify appropriate interventions - Support the continuous improvement of case management processes and procedures Qualifications: - Registered Nurse (RN) with a minimum of 3-5 years of case management experience, preferably in a healthcare or disability management setting - Strong clinical assessment and problem-solving skills, with the ability to work autonomously - Excellent communication and interpersonal skills, with the ability to build effective relationships with diverse stakeholders - Proficient in the use of technology and case management software - Knowledge of community-based health and social services an asset - Bilingual (English and French) is considered an asset
