DEFINITION:

Under general supervision, provides case management services to individuals and groups in various settings and provides discharge services to adults under the care of Grand Lake Mental Health Center Crisis Units. The Department Head for this position is the Crisis Services Director.

EDUCATION AND EXPERIENCE:

Bachelor’s degree in a mental health related field and ODMHSAS Case Management II Certification.

An expectation of core competencies in relation to individuals with co-occurring disorders.

 

KNOWLEDGE AND SKILLS:

Knowledge of the principles and practices of clinical social work, mental health and co-occurring disorders and psychology.

 

JOB DUTIES AND RESPONSIBILITIES:

Responsible and immediately supervised by the Crisis Services Coordinator.

This position is responsible and held accountable for the following duties:

· Plans jointly with Crisis Treatment Team to evaluate the appropriateness of discharge.

· Provides case management services to persons served at risk for placement in the community or other arrangements.

· Assist persons served in arranging for health care, housing, medication and other financial and educational resources, which are available.

· Maintains ongoing census status of contract hospital services and the Crisis Units.

· Participates as a member of a treatment team, with an aim of preventing recurrence of crisis.

· Consults with individual and community organizations on the improvement of services for persons served; provides advocacy with schools, courts, and other social service agencies as indicated.

· Arranges for appropriate services after discharge from the Crisis units for mental health and co-occurring disorders in accordance with team discharge plan.

· Has appropriate case management certification as needed.

· Responsible for completing crisis discharges during business hours and appropriate training of crisis staff on discharge process.

· Responsible for daily review and completing of ETPS measures related to crisis services including seven (7) day inpatient follow-up, eight (8) day outpatient follow-up and readmission within 180 days.

· Provides twenty-four (24) hour follow-up for GLMHC clients discharging from Wagoner Hospital or other affiliated hospitals.

· Completes all agency documentation of services in a thorough, accurate and timely manner

· Serves as liaison to assist in communication between Wagoner Hospital and GLMHC county offices.

· Regular and predictable attendance is an essential job requirement.

· Must meet agency standards of billable services and fiscal responsibility as designated by the Crisis Services Director.

· Performs other reasonably related duties as assigned by the Crisis Services Director or other management as requested.

· Participates in staff meetings

· Must meet “Satisfactory” expectations at any scheduled job performance evaluation. Quality of work will be evaluated through training, skill, or special purpose. Must be competent and adequate.
