About the role

GABA Telepsychiatry This is a remote position. A Remote Clinical Coordinator is responsible for developing care plans, coordinating patient care services, providing administrative support to the clinical team, and ensuring high quality patient experiences. Key Responsibilities: - Develop personalized care plans for patients based on individual needs and goals - Coordinate patient care services, including scheduling appointments, communicating with providers, and facilitating referrals - Provide administrative support to the clinical team, including data entry, file maintenance, and other clerical duties - Ensure high quality patient experiences by addressing patient questions and concerns in a timely manner - Maintain accurate and up-to-date patient records - Assist with the implementation of new clinical programs and initiatives - Collaborate with the clinical team to identify opportunities for process improvement - Participate in team meetings and trainings to stay up-to-date on best practices Qualifications: - 2+ years of experience in a healthcare, social services, or similar patient-facing role - Strong communication and interpersonal skills, with the ability to effectively interact with patients, providers, and team members - Excellent organizational and multitasking abilities, with the capacity to prioritize and manage multiple tasks - Proficient in using electronic medical record systems and basic computer software - Knowledge of relevant healthcare regulations and best practices - Commitment to providing high-quality, compassionate care to patients - Ability to work independently and as part of a team - Bachelor's degree in a relevant field (e.g., healthcare administration, social work, psychology) or equivalent experience
