Description

Position Summary:


The MDS Coordinator is responsible for the accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans in accordance with state and federal regulations. This role ensures proper documentation, coding, and interdisciplinary communication to support optimal resident outcomes and reimbursement accuracy.


Key Responsibilities:

  • Coordinate and complete MDS assessments, care plans, and support documentation within the regulatory timelines.
  • Collaborate with nursing, therapy, and other interdisciplinary team members to ensure accurate resident assessment data.
  • Monitor and maintain compliance with CMS and state guidelines.
  • Analyze and verify accuracy of coding to support appropriate reimbursement.
  • Educate staff on MDS processes and resident documentation requirements.
  • Participate in care plan meetings and quality improvement initiatives.

Requirements

Requirements: This is an onsite position.


Qualifications:

  • Current RN or LPN license
  • Previous MDS experience preferred (minimum 1 year in long-term care) and able to complete or maintain RAC-CT certification within 6 months of hire.
  • Knowledge of PDPM, RAI process, and federal regulations required.
  • Strong attention to detail and organizational skills.
  • Excellent communication and leadership abilities.
