Join our team and make a positive impact on the lives of others! We are looking for a Health Coach who will serve as the face of FreedomCare in the communities we serve in Illinois.

This is a hybrid position based out of our office in Chicago, IL with a required commute to the office of approximately 1x per month in addition to the time spent in the field.

Department & Position Overview

The Home Services Agency Supervisor is responsible for overseeing home service workers, ensuring the delivery of high-quality, non-medical support services to clients. This role includes conducting on-site visits, training home service workers, and ensuring compliance with Illinois Title 77, Part 245 Agency Code regulations.

Every Day You Will

Perform Client Assessments:

  • Conduct initial and periodic on-site visits to assess client needs and service quality.
  • Ensure that clients receive services in accordance with their needs and agency policies.


Supervision Of Home Services Workers

  • Oversee and provide guidance to home services workers, ensuring high-quality care.
  • Conduct on-site supervisory visits to observe and evaluate service delivery.
  • Provide direct feedback to caregivers based on performance assessments.


Training And Development

  • Ensure that all home services workers complete required training as mandated by Illinois Title 77, Part 245.
  • Conduct initial training for new hires, covering:
  • Client interaction and communication skills
  • Safety procedures and infection control
  • Recognizing signs of abuse, neglect, and emergencies
  • Personal care assistance techniques
  • Provide ongoing training and skill development sessions to ensure compliance and service quality.
  • Maintain training records for all home services workers.


Quality Assurance And Compliance

  • Ensure compliance with Illinois Title 77, Part 245 and other regulatory requirements.
  • Maintain accurate documentation of client services, training, and supervisory visits.
  • Implement quality control measures, including follow-up visits and client feedback collection.


Communication And Coordination

  • Serve as the main liaison between the agency, clients, families, and home service workers.
  • Address and resolve client complaints and concerns effectively.
  • Coordinate with healthcare professionals and case managers as necessary.


Administrative Duties

  • Assist in the development and enforcement of agency policies and procedures.
  • Participate in staff meetings and contribute to service improvement initiatives.
    • Ensure all documentation, including client records and training logs, is up to date.
Ideal Candidate Will Possess

  • High school diploma or equivalent (associate or bachelor’s degree preferred).
  • Experience in home services, healthcare, or a related field.
  • Supervisory experience and knowledge of caregiver training programs preferred.
  • Familiarity with Illinois Title 77, Part 245 regulations is plus.
  • Strong organizational, communication, and leadership skills.
  • Ability to conduct on-site visits and travel within the service area.
  • Previous field experience in a medical or case management environment, preferred
  • Experience with data input, case review/case note entry; Salesforce experience is a plus
  • Hands-on experience in home care, preferred
  • Working knowledge of ADL/ IADL assessments
  • Valid driver’s license with an insured vehicle for travel within the state
  • Excellent verbal and written communication, organizational and time management skills
  • Strong critical thinking and problem-solving skills
  • Proficient in Microsoft (i.e., Word, Excel, Outlook)
  • Excellent training skills
  • Team-player mentality

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $20.00 and $24.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
