Here’s what you’ll be doing:
- Serves as a central point of contact for clients requesting assistance from the Community Health Department and as a liaison between the Medical/Community Health staff, Family Services and other tribal programs on behalf of clients needing assistance.
- Assesses client needs, including medical, emotional, psychosocial, and social determinants of health.
- Develops individualized care or navigation plans and collaborates with interdisciplinary teams both within FCP and outside organizations and healthcare systems.
- Facilitates communication between clients, families, internal and external healthcare providers, and other key resources.
- Helps to identify and address barriers to care such as transportation, housing, insurance, appointment availability, financial challenges or other cultural barriers.
- Documents all client visits into appropriate documentation systems following the FCPCHD documentation policies. Ensures confidentiality of all records and information.
- Coordinates referrals to internal departments and external community resources and follows up to ensure service delivery.
- Provides education and support around diagnosis, treatment options, and health system navigation.
- Advocates for client needs within the healthcare system and the community.
- Participates in Quality Improvement and Public Health Accreditation activities as requested.
- Attends meetings, trainings, events and in-services that may require occasional evening, overnight or weekend hours as assigned by supervisor.
Here’s what you’ll need to be successful:
- Associate’s or Bachelor’s degree in social work, public health, nursing, human services, or other related field
- Other healthcare professionals considered with minimum of 2 years of experience in a clinical, hospital, or community health setting.
- Strong understanding of the healthcare system and community resources.
- Excellent communication, problem-solving, and organizational skills.
- Prior experience in care coordination, case management, or patient navigation preferred.
- Must have and maintain a valid driver’s license and be legible for coverage under the Tribe’s insurance. Must show proof of immunizations required for FCP Health Division employees and successfully pass a background check and pre-employment drug screen.
