This is a RN weekend primary on call position from home, but it is not fully remote. In this role, you will provide phone triage and support, must be able to travel to patient homes/nursing facilities when needed, and deliver care for symptom management, active decline, and at times, support patients and families at the end of life—including during and immediately after death. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes.
This position will have a rotation of weekends as follows: Friday & Saturday (4:30pm – 12mid) weekend #1 and then Sunday & Monday (12mid – 8am) weekend #2 and vice versa, responding to patient and family needs as they arise. There is no assigned caseload or scheduled visits—care is driven by patient needs and can vary throughout the shift.
