📌 Job Opening: Field Executive (PMJAY Department) – Phoenix Group
📍 Vacancies & Work Location:
- 1 Vacancy – Surat
*🏢 Head Office: Ahmedabad*
💰 Salary: ₹15,000 per month + ₹2.5 per km (Travel Allowance)
📝 Job Responsibilities:
- Daily field visits to 5 patients
- Collect patient statements, AVR, and required documents
- Coordinate with hospitals and patients under the PMJAY Department
- Ensure timely, accurate data collection and reporting
📅 Training Details (Non-Paid):
- 1 day training at Head Office (Ahmedabad)
- 6 days on-field training at assigned locations
📄 Job Type:
- Contractual / Contract-based position
📍 Coverage Area:
- Candidate must be willing to cover assigned districts as per the allocated location (Vadodara or Surat).

- Once you start this work, you cannot work with any other company or hospital during the contract period.
