Salary Range:

Requirements
- A minimum of a bachelor's degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, Human Services, or related field.
- At least 1 year of experience in Child Welfare related field preferred
Summary
This position is responsible for developing a meaningful and comprehensive transition plan in order to ensure healthy and safe transitions between living situations for Children placed in out of home care, pursuant to lead Agency policy, Children & Families Operating Procedure (CFOP) and Chapter 39 of the Florida of Statutes. Transition must be accomplished according to a plan that involves cooperation and sharing of information among all persons involved, respects the child’s developmental stage and psychological needs, ensures the child has all of his or her belongings, allows for a gradual transition from the caregiver’s home, and, if possible, allows for continued contact with the caregiver after the child leaves.
This position will work directly with our Quality Parenting Initiative (QPI) Liaison/Intake and Permanency Manager (IPM), and FRC Legal Department to ensure adherence to Citrus policy regarding Transition Staffings, Permanency Staffings, New Child Staffings, Intake Coordination, and Case Plan Conferencing.
The Transition Planning and Permanency Specialist (TPPS) serves as the Agency’s Point of Contact for transition planning and New Child Staffings for the corresponding Lead Agency (POC). POC duties will be divided as arranged between the Intake and Permanency Manager and (IPM) and the TPPS, by their direct supervisor.
2. Develops and implements a tracking system to ensure completion of all staffings for planned and unplanned transitions within the necessary timeframes dictated by CFOP and Lead Agency policy.
3. Develops and maintains this spreadsheet and complete all other required agency specific documentation to include transition plans and other documents filed with the Court.
4. Provides a tracker of all planned and unplanned Transition Plans to the Director of Full Case Management and Director of Quality of Assurance on a monthly basis.
5. Coordinates transition plan meeting with all necessary parties.
6. Facilitates transition plan meeting as required by CFOP and Lead Agency Policy.
7. Facilitates other transition tasks, including comfort calls as outlined in the Lead Agency policy and CFOP.
8. Assists in development of the FRC Transition Plan Protocol.
9. Ensures compliance with the FRC Transition Plan Staffing Protocol and New Child Staffing Protocol.
10. Works collaboratively with the Intake & Permanency Manager (IPM), FRC Legal Department and Case Management Department.
11. Cross-trains with all the duties of the IPM so as to ensure continuity of services in the absence of the IPM. Such duties include: Intake coordination, QPI (Quality Parenting Initiative) Staffings, Case Plan Conferencing, and Permanency Staffing. The description of such duties are hereby incorporated by reference.
12. Covers shelter hearings, if needed.
13. Participates and testifies in court hearings, if needed.
14. Trains all Agency’s full case management department on the Transition Plan Staffing Protocol and New Child Staffing Protocol.
15. Facilitates and tracks the Agency’s New Child Staffings.
16. Provides a tracker of all the Agency’s New Child Staffings, on a monthly basis.
17. Collaborates with the Full Case Management Department to ensure all necessary documentation and information gathering is completed for the New Child Staffing.
18. Collaborates with the Lead Agency in facilitating the New Child Staffings.
19. Provides a tracker of all New Child Staffings to the Director of Full Case Management and Director of Quality of Assurance on a monthly basis.
20. Documents all staffings in FSFN within 48 hours as required by CFOP and Lead Agency policy.
21. Attends all meetings as assigned by direct Supervisor including but not limited to Lead Agency Meetings concerning Transition Plans and Permanency.
22. Ensures that all programmatic and administrative practices comply with the highest relevant legal, professional, and ethical standards.
23. Makes himself/herself available after hours and on weekends, as needed.
24. Participates in the agency’s Quality Assurance/Quality Improvement Program
25. Contributes to the understanding and acceptance of cultural diversity within the agency and the community;
26. Complies with all relevant TJC standards and regulations.
27. Utilizes the agency’s resources in a fiscally conservative manner.
28. Performs other duties as assigned by Supervisor
