</div><div>Executive Home Care of Omaha is a growing home care agency led by a leadership team with a track record of scaling small companies into major businesses. We are looking for a seasoned social worker or care coordination professional who wants more than a job — someone ready to help shape an organization and grow with it.<br><br>
</div><div>This role is about relationships, not sales. You will work with local hospitals, skilled-nursing facilities, and rehab centers to connect families with the right home care support, coordinate client transitions, and work alongside our care team on scheduling and caregiver management.<br><br>

<strong><br></strong><strong><u>Responsibilities</u></strong><strong><br></strong><br>
</div><div>• Build and maintain referral relationships with SNF discharge planners, hospital social work departments, ALF directors, memory care units, and hospice teams<br><br>
</div><div>• Guide prospective clients and families through our care options<br><br>
</div><div>• Partner with leadership on community outreach and referral network development<br><br>
</div><div>• Support client onboarding, care plan coordination, and caregiver scheduling<br><br>

<strong><br></strong><strong><u>Qualifications</u></strong><strong><br></strong><br>
</div><div>• 5+ years of experience in social work, discharge planning, or home health coordination<br><br>
</div><div>• Established relationships within the local medical and social services community<br><br>
</div><div>• Licensed Social Worker, or equivalent credential preferred<br><br>
</div><div>• Strong communicator with families, referral partners, and care teams<br><br>
</div><div>• Comfortable in a small, fast-moving organization where roles evolve<br><br>
</div><div>• Team player who thrives in a collaborative environment<br><br>
