About the role

Position Summary: The Schedulers primary function is to facilitate scheduling of home visits for the field staff each day and assist with the coordination of patient care. Ensuring geographic scheduling is adhered to; increasing efficiency and enhancing staff satisfaction and improved and continuous patient care. Facilitates appropriate staff schedules to meet Company goals and objectives as well as to meet regulatory guidelines. Interacts with team members in a positive manner and while meeting set goals and objectives.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

Schedules approved home visit frequency for all field staff each day and assists with the coordination of patient care. Per Qualified discipline/Clinical Manager, ensure visits are allocated appropriately across all disciplines; RNs, LVNs, PT, PTAs, OT and COTAs, ST, HHA & MSW. Monitors and tracks staff visit load utilizing area assignments, status (full-time versus per diem), skill mix (RN versus LVN; OT versus COTA etc.) and discipline/Clinical Manager approved visit frequency. Projects schedule as ordered on the Plan of Care as well as when orders are processed for frequency changes etc. Assists Clinical Manager with reassignments in situations where staff calls in sick, on vacation, on leave and as the need arises to accommodate visits that need to be scheduled that day. As scheduling occurs, ensures that visits are geographically scheduled to reduce drive time and to enhance staff satisfaction and patient care. Schedules all new referrals to the appropriate clinician based upon location of the patient, clinician service area and availability. Verifies visit authorization is available and schedules accordingly. Utilizes EMR for all patient scheduling and data entry is done directly into system to reduce duplication and provide field staff with accurate information in a timely manner. Assists with scheduling compliance related to regulatory and policy guidelines; i.e. 14-day HHA supervisory visit, 30-day therapy re-assessment & LVN supervisory visits, 5-day recertification window visits etc.


Communication

Ensures all full-time staff are meeting productivity standards and works with Clinical Manager/Branch Manager when productivity standards are not met. Responsible for communicating with patients and staff as schedule changes occur, and updates EMR accurately and timely. Notify all disciplines when patient is transferred and when returns to agency and tracks and schedules Resumption of Care (ROC).

POSITION QUALIFICATIONS 1. High school graduate 2. AS degree/college degree preferred in administrative/business degree 3. Health Care experience preferred 4. Comfortable and proficient with computer use, i.e. Microsoft office suite as well as electronic medical record 5. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

 SKILLS REQUIRED 1. Ability to organize, prioritize and multi-task 2. Organized and a resourceful self-starter, strong and possess the ability to work in a team. 3. Excellent observation, verbal, written and interpersonal communication skills
