<strong>About the Role</strong><br> We are seeking a motivated and patient-centered Nurse Practitioner or Physician Assistant to join our growing clinical team in Melbourne, Florida. This role focuses on delivering high-quality primary care services in post-acute and long-term care settings, including Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs).<br><br>

<strong>What You’ll Do<br></strong><br>
</div><ul>
<li> Provide comprehensive primary care to adult and geriatric populations </li>
<li> Manage acute and chronic medical conditions </li>
<li> Perform routine and follow-up visits in SNF and ALF settings </li>
<li> Collaborate with interdisciplinary teams including nursing, therapy, and administration </li>
<li> Document encounters accurately and efficiently within the EMR </li>
<li> Participate in quality improvement and compliance initiatives </li>

<strong>What We’re Looking For<br></strong><br>

<li> Licensed Nurse Practitioner (NP) or Physician Assistant (PA-C) </li>
<li> Active Florida license</li>
<li> Experience in primary care, geriatrics, SNF, or post-acute care preferred </li>
<li> Strong clinical judgment and decision-making skills </li>
<li> Ability to work independently while maintaining collaboration with supervising physicians </li>
<li> Comfort with technology and electronic medical records </li>
