CNA - Full Time & Part Time 

The CNA is accountable for the delivery of quality home care services to the program's patients in conjunction with physician's directions, company and industry standards, and insurance guidelines as assigned by a registered nurse. The CNA is supervised by a licensed nurse to deliver quality home care services to the patients. CNA's work under the supervision of the Home Care RN and Health Care Coordinator (RN) and under the indirect supervision of the Director of Activities. The CNA will provide and assist participants of EveryAge PACE with activities of daily living, restorative, and supportive care. Assists with organization and implementation of activity program. Monitors health status and provides a safe environment for participants. Functions as a member of the Interdisciplinary Team.

Essential Duties and Responsibilities: 

1. Make patient visits to perform health care services, documents patient activities and appearance, and reinforces patients' and their families' appropriate health care techniques and principals to enhance patient recuperation and/or development of patient skills in self-care under Care Plan and Registered Nurse or Therapy Director.

2. Carries out physician-approved care plans and nursing orders, as well as, prepares pertinent paper and chart work in order to provide effective, professional home care to patients and their families in compliance with federal, state, and local guidelines. Follows the written patient care instructions as prepared by the registered nurse.

3. Maintains communication with the Home Care Coordinator RN or Clinical Supervisor and Registered Nurses to discuss the treatment plan and changes in assignment on a regular basis.

4. Attends and participates in regularly scheduled meetings to discuss patient care, develop existing skills, or to learn new health care techniques and practices. Attends a minimum of twelve hours of in-service instruction per calendar year.

5. Observe patient's progress and communicates to the Home Care Coordinator RN or Clinical Supervisor discipline observations that may necessitate changes in patient care to facilitate the patient's recovery.

6. Follows a weekly schedule of patient visits based upon the various care assignments as assigned by the Home Care Coordinator RN or Clinical Supervisor to ensure visit quota requirements are met.

7. Performs all duties relative to the Agency's definition of a standard visit.

8. Assists participants and/or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care, and toileting) as assigned or needed.

9. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Director of Activities.

10. Monitors and assists participants with meals at dining table.

11. Assists throughout the day in maintaining clean and orderly day rooms, kitchen, and personal care areas.

12. Accompanies participant to medical and other appointments.

13. Performs and/or assists with laundering of items, including clothes for participants.

14. Assists the Drivers with loading and unloading of participants from vehicles. Travels with Drivers to act as an escort for participants as needed.

15. Demonstrates dependability through consistent compliance with scheduled work hours.

16. Participates in the activities of the Interdisciplinary Team as needed and carries out the plan of care as directed.

17. Notes changes in participants’ physical and emotional health and communicates this information to the appropriate staff person.

18. Maintains participants’ medical records and fulfills EveryAge PACE charting and reporting requirements.

19. Assists the Transportation department with participants’ take home supplies.

20. Participates in and supports Quality Improvement initiatives.

21. Maintains safe working environment. Follows all EveryAge PACE program policies and procedures and Occupational Safety and Health Administration (OSHA) guidelines.

22. Acts within the scope of his or her authority to practice.

23. Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families.

24. Handles potentially infectious specimens with appropriate biohazard precautions and practices Standard Precautions.

25. Participates in continuing education classes and any required staff training and meetings.

26. Other duties as assigned by supervisor.

 

Minimum Education: 

High school diploma or equivalent/ Certified Nursing Assistant (C.N.A).

Requirements of C.N.A are current certification as Certified Nursing Assistant in the State of North Carolina required,

Cardio Pulmonary Resuscitation (CPR) certification required.


Minimal Experience:

A minimum of one year of experience in working with the frail or elderly population required.
