Qualifications:
- Associate's degree in Nursing required. Bachelor's degree in Nursing preferred
- One year of experience in home health care, mental health nursing or hospital
setting.
- Strong clinical and communication skills.
- Ability to travel within the service area, provide valid driver’s license and automobile insurance.
- Ability to exercise independent judgment.
- Must possess knowledge and understanding of co-occurring disorders (mental health and substance abuse); stages of change, phase of recovery and in motivational interviewing enhancement.
Supervisor Duties and Responsibilities:
1. Assures training and supervision of Health Services staff assigned to assure competency of skills and a high level of performance.
2. Assures that ongoing supervision/coaching is met for all staff assigned.
3. Assures that performance evaluations are completed at intervals stated in policy.
Clinical Duties and Responsibilities:
4. Collaborates as part of an interdisciplinary team in assessing, planning, implementing, and evaluating services for psychiatric, co-morbid and/or co-occurring psychiatric and substance use disorder consumers.
5. Completes nursing assessments to determine the needs of individuals for medical assistance, takes clinical histories and refers individuals to a physician/pediatrician/specialty physician when appropriate. Monitors assessments and screenings to assure findings are integrated in the care plan.
6. Participates in initial care plan development including specific goals for all enrollees.
