CRi is seeking a Nurse Coordinator for our Richmond, VA programs!
The Nurse Coordinator assesses and monitors the individual's on-going health care needs in all Richmond ICF programs, plus monitors and clinically directs the LPNs assigned to the ICF's to ensure the delivery of quality nursing care. The Nurse Coordinator collaborates with interdisciplinary team members, including the Medical Director, to coordinate medical treatment. They promote staff development regarding safe medication practice and provide training with a focus on health and wellness.
Principal Duties and Responsibilities:
- Provides general nursing care services within the legal scope of nursing practice and standards developed by the profession, Nurse Practice Act, and regulatory agencies.
- Uses professional nursing processes, including assessment, planning, implementation, and evaluation, while carrying out nursing responsibilities.
- Monitors and clinically directs LPN nurses and ensures delivery of quality nursing care at the ICF programs.
- Monitors compliance with regulatory agencies and licensure requirements.
- Assesses health care needs of consumers through a variety of routine interdisciplinary assessments, and develops, implements, and evaluates individualized nursing and health care plans accordingly.
- Assesses candidates for admission in the ICF. Coordinates health care requirements.
- Responds to emergency health care needs of the consumers. Participates in the on-call nursing rotation.
- Provides consultation to CRi staff regarding health/medical concerns, as requested.
- Provides training to CRi staff regarding medication management, infection control and other health/medical related subjects.
- Collaborates to develop guidelines/protocols for health/nursing/nutritional needs.
- Participates in interdisciplinary planning meetings and recommends changes in nursing and health care programs and/or in the care of consumers.
- Provides health care instruction and information to individuals and staff relating to the promotion of health and prevention of management of illness, disease, injury or disability.
- Perform special projects, as assigned by the Director of Nursing.
Requirements
Qualified Candidates Must Have:
- At a minimum, a graduate of an accredited RN program.
- A Current, unencumbered, active license to practice as an RN in the Commonwealth of Virginia or a state which participates in the Nurse Licensure Compact.
- At least two (2) years of experience directly related to the duties and responsibilities.
- At least one (1) year of experience working with ID/DD population preferred.
- Ability to read, write, speak, and understand the English language.
- Ability to make independent decisions.
- Ability to deal tactfully with personnel, individuals, family members, visitors, government agencies/personnel, and the general public.
- Leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
- Willing to seek out new methods and principles and be willing to incorporate them into existing nursing practice.
- Ability to relay information concerning an individual's condition.

Supervision Exercised:
- Supervises Licensed Practical Nurses in ICFs.
