Description
Provide the highest degree of quality care and services to a consistent group of residents
and their families in our assisted living and memory care neighborhoods. This role includes
providing hands-on care, physical and emotional support as outlined in each resident’s
Personal Service Plan (PSP) while maintaining a safe, comfortable home-like environment.
The Caregiver is responsible for demonstrating all elements of wellness, remaining in
accordance with federal, state and local standards as well as Twenty/20 Management
policies to promote the highest degree of quality care and services to our residents.
Caregivers with a CNA endorsement/credential are expected to maintain credentials and
follow certification standards along with community policies and expectations. They may
also have additional or enhanced duties assigned accordingly
Requirements

-Must be courteous, considerate and understanding when dealing with Residents, team
members and visitors.
-Ability to work closely with others.
-Must pass required background checks and meet all regulatory health and training
requirements.
-Because staffing levels in senior living are census- and care-needs driven, work hours and
schedules are not guaranteed and may be modified, increased, or reduced at any time based
on operational and business needs.
Summary
RESPONSIBILITIES – Included but not limited to:
-Reports shift update in communication log ensuring reporting of all incidents to the med tech/nurse
-Report all incidents and accidents to the med tech and/or RCD immediately
-Reports all unsafe and hazardous conditions/equipment immediately to the executive director or designee
-Participate in the development of the Personalized Service Plans as requested by administration
-Knows and complies with all residents’ rights
-Checks all assigned residents daily for cleanliness, appropriate clothing, any continence issues, and change in condition
-Initiates, assigns, and assists with activities as appropriate
-Uses proper sanitation procedures and universal precautions
-Checks resident apartments daily for any hazard
-Completes activities of daily living for assigned residents as per the Personalized Service Plan
-Completes resident laundry as per schedule
-Responds to security system and resident call alarms immediately taking appropriate action when responding
-Complete all training as assigned
-Complete all state, federal, and company training as assigned
-Participates in rounds with off going and on coming shifts, including necessary documentation
-Completes the daily Caregiver assignment in the appropriate platform
-Clocks in and out as appropriate
-Follows attendance policy
-Creates and maintains a warm professional environment
-Answers phone as appropriate
-Attend staff meetings as scheduled
-Serves meals in dining room as assigned
-Promotes and ensures a pleasant dining experience
-Assists with dining room set up and clean up
-Follows resident’s diet as indicated on the Personalized Service Plan
-Provides room service as needed
- Must fulfill all duties assigned, to include Life Enrichment & other department support as needed
-Demonstrate company core values at all times
-CNAs must follow reporting/documenting and tasks per certification guidelines along with community policy and task assignment
-Depending on experience and performance standing – may be assigned job lead duties and act as support for peers as needed and may rotate duties with other leads

Ability to manage intermittent or high-volume periods of repetitive tasks and sustain extended physical activity, including sitting, standing, walking, reaching, climbing, bending, lifting, grasping, pushing, pulling, and other weight-bearing and maneuvering techniques as needed

Ability to work in environments with low or bright lighting, loud noise, strong odors, variable temperatures (hot or cold), and potential exposure to infectious diseases, bloodborne pathogens, and other health-related risks

Efficiently and effectively manage unpredictable schedules, distractions, changes of pace or priorities, on a regular basis
DRIVER RESPONSIBILITIES/CLEAN MVR REQUIRED:
-Meet general and specific Key Performance Indicators and associated industry or community metrics as leadership assigns and deems defining of success
-Adjust to changing or evolving metrics accordingly
