</div><div>We are seeking a dependable, detail-oriented individual to join our team as an Intake Coordinator – Home Health. This role supports both patient intake coordination and medical records administration within a fast-paced home health environment.<br><br>

<strong>Key Responsibilities<br></strong><br>
</div><ul>
<li> Coordinate patient intake for home health services, including gathering and verifying patient information </li>
<li> Prepare and maintain patient medical records and documentation </li>
<li> Communicate with patients, clinicians, and physician offices as needed </li>
<li> Assist with scheduling and coordinating home health visits for clinical staff </li>
<li> Follow up on physician orders and required documentation to ensure completeness and compliance </li>
<li> Enter and update information accurately in the electronic medical record system </li>
<li> Support general administrative and phone duties as needed </li>
<li> Maintain confidentiality and comply with HIPAA and agency policies </li>
