About the role

Description

The Intensive Support Supervisor will assist in developing person-centered support plans that address the needs of families and individuals. They will implement intensive support services that advance each person’s support plan of care. Intensive Support Supervisor will provide direct care to individuals in the program and ensure appropriate operational support. The intensive Support Supervisor will also monitor and evaluate program progress, participant satisfaction and staffing.

 

POSITION RESPONSIBILITIES

  • Understands and follows the standards set forth by OPWDD and OMH, including specifics of workflows, program requirements and overall best practices.
  • Maintains their first allegiance to the people they work for by demonstrating respect and supporting problem solving skills.
  • Has solid knowledge and understanding of person-centered planning and service delivery.
  • Assists the program coordinator in designing support activities that reflect people’s interests, choices, and strengths.
  • Delivers support sessions and related services as outlined in each person's intensive support plan of care.
  • Assists in the development (up to and including attending Life Plan or Plan of Care meetings), implementation and on-going evaluation of service plans that are based on the person’s preferences, needs and interests.
  • Ensures necessary eligibility is in place for the appropriate authorization of services.
  • Collaborates with Managed Care affiliates and Care Coordinators to ensure appropriate service utilization.
  • Acts as a liaison between the program and the Family Supports and Services Committee, ensuring ongoing communication and participation during annual reviews.
  • Coordinates participation of training for staff that is relevant to on-going support of people with developmental disabilities and mental illness and emotional disturbance. 
  • Oversees staff by providing supervision, support with client scheduling, training, and monitoring overall delivery of service and program compliance.
  • Completes accurate documentation within the required timeframe including reviews of daily notes, monthly summaries, service billing and FSS Quarterly Reporting.
  • Assesses those supported and communicates any program recommendations that best support the person, such as frequency of service, referrals and changes to behavior supports. 
  • Facilitates support sessions that are aligned with treatment plans and Life Plans; communicates any changes that are needed to plan to Program Coordinator and/or Care Manager.
  • Coordinates services and ensures coverage for cancelations. 

Requirements

Education and Experience

  • Five years of experience working with adults with disabilities and complex behaviors; OR
  • Associate’s degree with two years of relevant experience; OR
  • Bachelor Degree in Health and Human Services with one year of relevant experience.

Skills, Knowledge and Abilities

  • Knowledge of developmental disabilities and serious mental illness
  • Good oral and written communication skills and basic computer skills. 
  • The ability to read and write case notes as well as understand written directions.
  • The ability to self-initiate, organize, schedule and analyze tasks.

Certifications be or become

  • CPR/First Aid certified.
  • Full SCIP-R certified.

Be authorized as an Agency Approved Driver by 

  • Holding and maintaining a valid NYS Driver’s License.
  • Maintaining an approved driving record according to Agency Policy and Procedure. 

Physical and Mental Requirements

  • Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs as well as the ability to perform all required movements required by the SCIP-R curriculum. 
  • Lifting up to fifty (50) pounds.
  • Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines
  • Staying awake and alert while on duty.
