JOB DESCRIPTION: CASE MANAGEMENT DIRECTOR
REPORTS TO: Executive Director
PRIMARY FUNCTION: Coordinate Program Services in the ICF-IID Services and the Medicaid Waiver Services.
RESPONSIBILITIES:
1. Supervise Medicaid Waiver and ICF-IID Case Managers and Case Manager Assistants to develop, coordinate and monitor all people supported programs related to the Support Plans
2. Supervise ICF-IID Therapy Coordinator to monitor people’s physical therapy plans and equipment and to provide staff training on physical therapy plans
3. Supervision Program Specialist for Medicaid Waiver and ICF-IID to coordinate behavior program plans
4. Be the contact for the ICF-IID contract supports (Speech Language Pathologist, Occupational Therapist, Dietitian, PT)
5. Monitor Implementation of Programs
6. Be the lead representation for all surveys, develop plans of correction and ensure compliance
7. Participate in Utilization Review for ICF-IID
8. Coordinate and document family /conservator contact for people supported
9. Assist with internal investigations for ICF-IID and DIDD investigations
10. Communicate with other departments and Directors regarding people’s needs, updates or changes in people’s programs or needs and other issues or concerns that affect the programs
11. Perform other duties as assigned
QUALIFICATIONS
Bachelors Degree in Human Services Area
A minimum of 3 years of experience in a job related area.
Ability to be insured as a vehicle driver by agency’s insurance program.
A valid Tennessee Driver’s license.
Good knowledge of basic English sufficient to read instructions, keep records and write reports.
Successful completion of background and reference check.
