Roles & Responsibilities
Role Purpose
The Full-Time AgeWell Outreach & Education Coordinator is responsible for community outreach, public information, and preventative health education across the six-county service area, ensuring alignment with the Area Plan and Older Americans Act (OAA) Title IIIB and IIID requirements.
This position is field-based and execution-focused, with clear expectations for county coverage, stakeholder engagement, and reporting.
Time Allocation (Required)
The Full-Time Coordinator must maintain the following workload distribution:
- 50% – Preventative Health Education (Title IIID)
- 50% – Outreach & Public Information (Title IIIB)
Within this workload distribution:
- 60% – Direct Outreach & Education Activities
- 40% – Planning, Administration, Reporting, and Training
This allocation is monitored weekly and evaluated as a core performance standard. Time breakdown must be completed on excel file and submitted back to supervisor every Friday.
Excel file must be complete, time inserted before UKG submission. UKG submission should be supported by excel file completely. If not then UKG will be denied and sent back.
Preventative Health Education Responsibilities (50%)
The Coordinator will:
- Deliver evidence-based preventative health programs to older adults
- Ensure each county receives equitable quarterly coverage

- CoAction provides services in Lake, Porter, Jasper, Newton, Starke and Pulaski counties
- Support implementation of Title IIID programming across all six counties listed above
- Class session may be reviewed by Age Well supervisory team on an ongoing basis as programs are added to Preventive Health
- Facilitate classes focused on:

- Chronic disease self-management plan
- Matter of Balance
- WISE
Execution expectations:
- Maintain a rolling 6–12 month county-based education schedule with a minimum of 2 classes, 3 preferred
- Maintain updated calendar for Preventive Health and Coordinated Outreach efforts
- Independently manage logistics, locations, materials, and attendance
- Track participation and outcomes
Outreach & Public Information Responsibilities (50%)
Outreach (Targeted – Title IIIB)
Outreach activities are targeted, intentional, and individualized efforts designed to identify and engage older adults who are:
- Socially or economically disadvantaged
- Living in rural or underserved areas
- Members of minority populations
- Individuals with limited English proficiency
- Isolated or not currently connected to services
Responsibilities include:
- Locating and engaging individuals not already connected to services
- Disseminating information about aging, nutrition, health, and support services
- Assisting individuals in navigating systems of care
- Making warm referrals to appropriate programs and partners
Activities include:
- One-on-one or small group engagement
- Navigation assistance and referrals
- Relationship-based outreach with community partners
Public Information (Broad Awareness – Title IIIB)
Public information activities are broad awareness efforts, not individualized outreach, and may include:
- Community presentations
- Outreach booths at fairs and expos
- Educational sessions for community groups
- Distribution of materials at public events
Each public information activity is recorded as a distinct activity, with estimated audience reach documented for NAPIS reporting
Reporting Requirement:
Title IIIB: Activities and estimated audience counts documented for NAPIS. Each public information activity is recorded as one activity, with audience reach documented where required.
County Coverage Responsibilities
The Coordinator is responsible for documented outreach and education presence in:
- Lake
- Porter
- Jasper
- Newton
- Pulaski
- Starke
Minimum expectations:
- Outreach presence in each county every quarter
- Education activities distributed equitably across counties Education classes in each county every quarter

- Stakeholder engagement documented by county
Targeted Community Stakeholder Outreach
The Coordinator will conduct ongoing outreach with:
- Senior centers
- Assisted Living
- Healthcare providers
- Faith-based organizations
- Housing providers
- Community-based organizations
- Local advisory councils and coalitions
Responsibilities include:
- Identifying priority stakeholders by county
- Scheduling outreach meetings or presentations
- Tracking engagement and follow-up
- Aligning outreach with Area Plan priorities and underserved populations
Materials Management
The Coordinator must independently:
- Gather, organize, and transport outreach materials and incentives if applicable
- Maintain sufficient inventory of printed and educational materials
- Ensure materials are prepared before events and classes
Attendance & Professional Standards
The Coordinator must:
- Show up as scheduled for all outreach and education activities
- Arrive prepared, on time, and ready to engage
- Communicate proactively regarding conflicts or barriers
- Represent CoAction professionally at all times
Reliability and follow-through are core performance expectations.
Reporting & Accountability
- Weekly reporting required, including:

- Accurate documentation for:

Requirements
- 1 year experience in related field.
- Organizes and prioritizes multiple tasks
- Ability to learn the fundamentals of programmatic policies and operating procedures.
- Understanding of the agency’s client populations.
- Need to be able to lift/carry up to 40lbs
- Communication: Excellent verbal and written communication skills to effectively empathize with clients, and collaborate with team members and external partners
- Cultural Competence: Sensitivity to cultural differences and an inclusive approach when working with individuals from marginalized or stigmatized groups
- Resourcefulness: Capacity to identify and leverage available resources, services, and programs to meet the needs of the target population
- Technology – Proven abilities, knowledge, and acumen to keep pace with current applications, programs, and platforms
