Choice Hospice is seeking a compassionate and experienced Nurse Practitioner (PRN) to provide high-quality, patient-centered care to individuals with life-limiting illnesses. This role is ideal for a clinician who thrives in a flexible, fast-paced environment and is passionate about hospice and palliative care.
The Nurse Practitioner plays a key role in supporting the interdisciplinary team, guiding patient care, and ensuring comfort, dignity, and quality of life for patients and their families.
Key Responsibilities:
- Collaborate with the Interdisciplinary Group (IDG) to ensure appropriate evaluation and palliative treatment of terminal illnesses
- Conduct Face-to-Face visits in patient homes as required
- Assess patient conditions and communicate findings to hospice physicians
- Educate patients and families on disease progression, symptom management, and end-of-life care
- Provide guidance and clinical support to hospice staff as needed
- Coordinate with attending and consulting physicians regarding care plans
- Maintain current knowledge of hospice eligibility criteria, pharmacology, and palliative care best practices

Requirements:
- Current RN and Advanced Practice Nurse (APN/NP) license in the state
- Active AHA CPR Certification
- Valid Driver’s License and Auto Insurance
- Experience with Homecare Homebase (HCHB) preferred
- Hospice or palliative care experience strongly preferred
