The Hospice Field RN provides compassionate, patient-centered care to individuals with life-limiting illnesses in their homes or care settings. This role is focused on delivering high-quality end-of-life care that promotes comfort, dignity, and quality of life while supporting patients and their families.
You’ll work closely with an interdisciplinary team to coordinate care, manage symptoms, and guide families through the hospice journey.
What You’ll Do
- Conduct initial and ongoing patient assessments (physical, psychosocial, and environmental)
- Develop, implement, and update individualized plans of care
- Determine hospice eligibility and appropriate levels of care
- Educate patients and families on disease progression, medications, and end-of-life care
- Coordinate care with physicians and interdisciplinary team members (IDG)
- Supervise and provide clinical direction to LVNs/LPNs and Hospice Aides
- Document patient progress and maintain accurate clinical records
- Communicate changes in patient condition to the care team
- Participate in on-call rotation as assigned
What You’ll Need
- Active RN license
- CPR certification
- Valid driver’s license and auto insurance
- Minimum 2 years of hospice experience (preferred)
- HomeCare HomeBase experience (preferred)
- Strong clinical judgment and communication skills
