As a Home Health LVN/LPN, you’ll practice at the top of your license—delivering skilled, one-on-one care in patients’ homes. This role offers the autonomy to use your clinical judgment while making a meaningful impact on patient outcomes, comfort, and independence. You’ll work independently in the field, while staying connected to a supportive and collaborative care team.
What You’ll Do
- Partner with the RN to develop and implement individualized patient care plans
- Deliver skilled nursing care, including medication administration and wound care
- Educate patients and caregivers on treatment plans and disease management
- Monitor assigned caseload to ensure continuity, quality, and compliance with the Plan of Care
- Track patient progress, identify changes in condition, and communicate updates to the RN and physician
- Collaborate with the interdisciplinary team to ensure high-quality, coordinated care
What You’ll Bring
- Current LVN/LPN license (required)
- Valid driver’s license and auto insurance
- Current CPR certification
- Home health experience preferred
- Experience with HomeCare HomeBase (preferred)
