Job Purpose or Objective(s): As the Case Manager, you will be a source of social, educational, and spiritual support for individuals and families to develop skills and enhance quality of life. You will report to the Departmental Manager or Director.

- Bachelor’s degree in human service, social work, or education field and one [1] year directly related experience, OR five [5] years of directly related experience in lieu of education.
- Good social skills and ability to build trust and relationships.
- Good oral and written communication skills
- Basic knowledge of Microsoft Word and Excel
Responsibilities
You will contact clients to perform intake assessment and analysis.
Provide an overview to the client by describing the extent and nature of the services available to them.
Provide educational and social case management services to clients including setting goals and developing case plans
Review and monitor participant progress and provide resources, advocacy, and referrals.
You will manage case to ensure progress and adjust as needs change.
Develop working relationships with individual patient and family.
Make referrals to community agencies and conduct follow-up activities.
Be a patient liaison and advocate.
Perform other responsibilities as assigned.
