About the role
Job Summary
The Quality Control Technician manually indexes documents to the correct level as established per facility policy, minimizes duplicate records and overlap entries, verifies data integrity, and reviews the electronic document management system and the records contained within it for the appropriate image quality and indexing.
Essential Duties and Responsibilities
1. Indexing
-
Monitor the manual indexing queue of unassigned images
-
Prioritize the batches in the queue and retrieve the batches for processing
-
Review each electronic image in the batch
-
Determine the correct patient name, medical record number, account number, and document type for each image
-
Review and determine if the image should be:
-
Added as a new document
-
Replaced with an existing document
-
Inserted into an existing document
-
Deleted
-
Index the imaged record appropriately by selecting the correct patient, medical record number, and encounter
-
Consistently maintain established productivity requirements and maintain a 96% or greater accuracy rate
2. Quality Control
-
Determine the readiness of the batch for quality control
-
Locate the electronic record for verification against the paper record
-
Review each image in the batch and verify the following:
-
Correct patient name and account number
-
Proper document type
-
Image quality (readable, orientation, multiple sides, etc.)
-
Verify receipt of all interfaced documents (transcriptions) for each encounter
-
For each page scanned or interfaced with errors:
-
Rearrange out-of-order images within the electronic document
-
Relocate electronic images that are incorrectly filed in another department
-
Relocate electronic documents that are incorrectly filed in an encounter
-
Replace electronic images and documents that have unacceptable image quality or have been updated
-
Modify the document type or misfiled electronic documents
-
Generate an error log and forward to the Operations Manager
-
Understand and use the department computer system in an effective and proficient manner
-
Adhere to Chesapeake Regional Medical Center’s confidentiality policy for all information related to patients, family, hospital employees, physicians, and clients

Education

Minimum Required Education: High school diploma

Preferred Education: Associate’s Degree in Health Information Technology
Experience
-
3+ years in a Health Information Department in an acute care setting
Additional Qualifications
-
Extensive working knowledge of medical terminology and medical record content and format for inpatients and outpatients
-
Preferred candidates will hold a current RHIT certification
