JOB SUMMARY: (Brief Statement of Purpose)
Under direction and supervision of the Community Living Coordinator, supervises one licensed site or multi semi-independent sites which includes assessing, evaluating and supervising social services for individuals with developmental disabilities. Performs related work as required.
DISTINGUISHING CHARACTERISTICS:
This classification is responsible for coordinating applicable service, social, vocational, educational and community support/activities with designated Individuals. This classification is distinguished from other support classifications by its responsibility for planning, organizing, supervising and participating in medical, individual support plans and social service activities directly related to supporting Individuals.
DUTIES/RESPONSIBILITIES: (illustrative only- Work assignments may vary and will be communicated to the employee by the supervisor)
- Management of site(s) includes activities regarding providing support to Individuals and providing supervision to employees, including but not limited to: Scheduling shifts, assigning work, directing supervision of employee’s work and corrective action; maintaining licensing standards
- Coordinates, supervises and participates in the activities of a specialized program and maintains effective staffing patterns to ensure quality support;
- Communicates and interacts with Individuals on an individual and group basis, promoting individual choice, freedom, responsibility and support for Individuals;
- Provides direct support for Individuals, provides support to staff, reviews care environment and appropriateness of social services , and Individual changes as needed;
- Ensures a safe environment is maintained by developing and communicating procedures with Individuals, family members, support coordinators, visitors, staff and, management
- Observes Individual’s behavior and consults with primary care physicians, agencies and family/guardians regarding needed support;
- Evaluates support/functions and encourages Individuals to participate in decision-making, counseling, social, recreational, vocational, day and other activities identified in their individual support plan or person-centered plan;
- Coordinates transport and/or escorts/transports Individual to vocational, residential, social activities and related errands as needed;
- Assists in developing, recommending and implementing CGArc policies, standards, services and procedures
- Takes part in implementing individualized support plans or person-centered plans for Individuals and ensures plan directives are followed in compliance with State/Federal/local standards;
- Facilitates staff and administrative meetings as well as in-service training;
- Ensures proper administration of non-prescription and prescription medication (excluding IV medications) as prescribed by licensed medical professionals
- Serves as technical resource for staff and assists in resolving problems with the transfer of power and control from system to Individual scheduling changes, service coordination and delivery problems;
- Reviews progress notes and daily activities recorded in communication logs to ensure compliance with all regulations;
- Reviews incident reports and informs appropriate staff regarding incidents involving vocational, day, social, recreational, behavioral, personal living and community support problems;
- Ensures standards of care are maintained and monitors Individual’s mental and physical health to enable appropriate intervention/prevention or problems;
- Operates equipment to aid in the assistance and/or movement of incapacitated individuals;
- Assists in performing various activities including crisis intervention as well as physical management procedures as necessary;
- Maintains Individual records and prepares related reports and correspondence;
- Accountable for member and agency funds
- Develops and maintains working relationships with all team members, including Support Coordinators, family members, guardians, fiduciaries, advocates, etc.
- Working at multiple homes, locations, and departments at the discretion of the agency
- Ability to be proficient in agency used computer systems and software
- PERFORMS OTHER RELATED DUTIES AND ASSIGNMENTS AS REQUIRED.
KNOWLEDGE & SKILLS:
- Human behavior indicative of mental or physical disability;
- Functions, practices and goals of residential support, day and vocational programs;
- Safety precautions in transportation and Individual care;
- Applicable program, CGArc, State or Federal rules/regulations, and licensing requirements in the State of Arizona;
- Community resources and social service agencies;
- Psychiatric disorders, psycho tropic medications and their side effects;
- Establishing and maintaining effective working relationships with professional staff, Support Coordinators, Direct Support Professionals and community organization representatives;
MINIMUM QUALIFICATIONS:
- Must be at least 18 years of age (licensing requirement). If driving is preferred for the position, must be 21 years old.
- Have a clean driving record per agency insurance standards.
- Possess a valid Arizona driver’s license and be insurable under the agency’s insurance policy.
- High School diploma or GED. Six (6) months experience in human services/behavioral health field.
- May be required to drive personal vehicle and have acceptable vehicle insurance to be used for agency travel as indicated by specific assignment, if necessary.
- Be available by home phone, cell phone, etc. for easy access by agency personnel.
- Able to work flexible schedules; weekends, nights, days, evenings, partial shifts, 8/12/16 hour shifts.
MENTAL AND EMOTIONAL REQUIREMENTS:
- Ability to effectively communicate in English both in writing and verbally.
- Able to positively interact and develop rapport with individuals and their families, professional support staff, and various levels of staff from community agencies.
- Able to maintain a calm, non-defensive, supportive attitude at all times including crisis or potential crisis situations.
PHYSICAL REQUIREMENTS:
- Must be able to lift individuals.
- Must be able to climb stairs and assist individuals in moving household items if necessary.
- Must be able to assist individuals with household chores (involving many physical activities, including but not limited to kneeling, reaching, stretching, bending, etc. and the use of household cleaners).
- Must be able to endure extreme outdoor temperatures and sudden temperature changes.
- Must be able to demonstrate competency in the following areas:
- Operate an agency transport vehicle if requirements are met to drive
- Manual dexterity to keep documentation records
- Able to see and hear Individuals in order to monitor Individual activities
- Perform crisis intervention techniques to prevent behavior harmful to the individual or others which may require significant physical activity
- Assess and provide behavior management in crisis situations and call for assistance if needed
- Be able to physically perform First Aid/CPR, and CIT
OTHER REQUIREMENTS:
Licenses and certificates:
- Must be willing and able to attend training classes as scheduled
Special Notice Items:
- In-service training sessions are required.
- Due to Department of Economic Security/Division of Developmental Disabilities requirements, positions in the category are required to provide fingerprint card or be able to obtain fingerprint clearance
- Department of Labor regulation CFR Part 1910.1030 requires notification that this category may have a risk of exposure to blood borne pathogens. Hepatitis B vaccine will be provided upon request
