Role Overview
We are looking for a dedicated Case Manager to support patients throughout their treatment journey and coordinate closely with the clinical team.
Key Responsibilities
- Work closely with the Senior Resident and keep them updated on patient progress and key decisions.
- Take ownership of assigned cases and maintain a thorough understanding of each patient's history, treatment plan, and progress.
- Meet patients each morning and prepare relevant updates for IPD rounds.
- Support psychoeducation, counselling, family sessions, discharge planning, and case coordination under the guidance of the clinical team.
- Maintain accurate patient documentation and ensure timely communication between patients, families, and the clinical team.
- Identify concerns or changes in patient condition and escalate them to the concerned clinical professional.
- Act as a key coordination point to ensure continuity of care throughout the patient's treatment journey.
Eligibility
- MSW / Master's in Psychology, or
- Bachelor's in Psychology with 1–2 years of relevant experience
- Experience in mental health, rehabilitation, hospital, or psychiatric settings will be preferred.
- Good communication, coordination, documentation, and interpersonal skills.
Location Preference
Candidates residing in or around Greater Noida will be preferred. Candidates willing to relocate nearby may also apply.
Note: This is a full-time position requiring 6 days a week and 9 hours per day. Part-time opportunities are currently not available.
