About the Role

At Corus Medical, you’ll join a mission-driven, highly supportive team shaped by generations of care. As a PRN Nurse Practitioner, you’ll step in during key moments to deliver high-quality, patient-centered medical care across home health and hospice. Your presence helps stabilize patients, support families, and ensure continuity of care — often when it matters most.

Key Responsibilities

  • Perform comprehensive assessments, interpret findings, and develop appropriate treatment plans.
  • Complete Face-to-Face (F2F) visits for certification and recertification of home health and hospice services.
  • Diagnose, treat, manage medications, and order tests within New Mexico NP scope of practice.
  • Provide clear, compassionate education to patients and families regarding conditions and care plans.
  • Document accurately and promptly within the electronic health record.
  • Collaborate with physicians, nurses, therapists, and IDG team members to ensure cohesive care.
  • Support symptom management, chronic disease care, and acute changes to prevent unnecessary hospitalization.
  • Navigate varied home environments with professionalism, safety, and empathy.

Requirements

We’re Looking For

  • Current NP licensure in New Mexico; FNP, AGPCNP, or comparable certification.
  • Two or more years of NP experience preferred, especially in home-based or complex medical settings.
  • Strong clinical decision-making in the field with high independent judgment.
  • Exceptional communication and patient engagement skills.
  • Demonstrated empathy, follow-through, adaptability, and calm under pressure.
  • Integrity and disciplined documentation habits.
  • Ability to travel locally and work PRN hours based on patient and agency needs.

About the company

History The founders of CareM have maintained a family tradition of providing healthcare that spans nearly 100 years, from the sanitariums of the early days to the patient centered array of services offered today.
